Overview
This group programme supports people living with long-term physical and/or mental health conditions, as well as caregivers. Participants learn practical skills to improve confidence, manage health challenges, and support their overall wellbeing.
The programme focuses on:
Action planning and problem‑solving
Healthy eating and movement
Managing emotions
Communication and self‑advocacy
Working effectively with healthcare providers

Programme details:
Programmes run in person across Auckland and online, with daytime and evening options.
6-week programme: 1 session per week, 2.5 hours per session
4-week programme: 1 session per week, 90 minutes per session
Referral options:
GP, Nurse, Health Improvement Practitioner, or Health Coach

Our self-management education programmes are facilitated by experienced facilitators who specialise in supporting people to build confidence, self-management skills, and healthier lifestyle habits. Facilitators create an engaging and supporting learning environment where participants can share experiences, learn practical strategies, and work towards their personal wellbeing goals.
